EMS Certification Levels: EMR, EMT, AEMT, and Paramedic

A national model, with real state-level authority on top of it

The National EMS Scope of Practice Model, developed by the National Association of State EMS Officials with NHTSA/HRSA funding (most recently revised in 2019, with change notices through August 2021), defines four nationally-recognized certification levels and the minimum competencies for each:

Why "national model" doesn't mean "nationally licensed"

The National Registry of Emergency Medical Technicians (NREMT) administers the national certification exam most states use as part of their own licensing process — but it's each individual state EMS office that actually licenses a person to practice, and a state can add requirements, restrict a national-model skill, or require additional state-specific testing beyond the national baseline. A paramedic certified nationally through NREMT is not automatically licensed to practice in a different state without that state's own licensing action — this varies state to state and should never be assumed reciprocal without checking.

Why this matters for staffing and recruitment planning

A department's real EMS staffing capability is defined by which personnel hold current state licensure at which level, not just how many people attended an EMT course at some point — a lapsed certification or a certification that was never converted to state licensure doesn't count operationally, however well-intentioned.

For small and volunteer departments

Smaller and combination departments commonly staff at the EMR or EMT level specifically because that's the realistic training/retention commitment for part-time or volunteer members — that's a legitimate staffing model, not a lesser one, provided the department's actual call-type mix and mutual aid/transport arrangements are honestly matched to what that level can provide.
